Benefits & Financial Planning Launches New Website
The UCSF Benefits team in coordination with UCOP BRC/Payroll and UCLA Benefits/Payroll is now fully supporting benefits administration for employees at the Office of the President. Please take the time to check out the recently updated and enhanced website for benefits information specific to UCOP employees, HR administrators, and supervisors.
We’d like to thank Eva Hom, Dee Brown, and the folks under BRC/Payroll who have all worked very hard over the last months to support the transition. The Benefits team looks forward to supporting you in understanding and obtaining the full value of your UC sponsored benefits programs.
The following are frequently asked questions that may be helpful.
FAQs:- How do I contact Benefits?
A. Answers to many questions may be found on the benefits website. If you still have questions, contact benefits administration by calling 415-476-1400 or emailing benefits@ucsf.edu.
- How do I schedule a retirement counseling appointment?
A. Contact the main benefits office at 415-476-1400 or benefits@ucsf.edu. We have an office on the 5th floor of the Franklin building from which to provide these services.
- How do I request a retirement estimate?
A. There are several ways to get an estimate of your retirement benefit:
i. Log on to At Your Service Online to see a personalized estimate of basic retirement income and lump sum cashout based on your current payroll data.
ii. Use the Retirement Plan Benefit Estimator to estimate your retirement benefit based on data you collected from At Your Service Online.
iii. Review the Retirement Handbook for additional information.
- How do I make a change to my benefits?
A. After initial enrollment, changes may only be made during Open Enrollment or during a Period of Initial Eligibility (PIE). Please review the Family Changes Benefits Checklist for more details. Changes outside of a new hire PIE or open enrollment require a UPAY 850. Once completed, send it to the BRC/Payroll for processing (exception: employees of OGC, EAP, and UC Press send these forms to their own HR dept.)
- Who do I contact if I am having an escalated problem when utilizing my health benefits?
A. Contact Sue Forstat, our Health Care Facilitator, at 415-514-3324 or sue.forstat@ucsf.edu. For information about our Health Care Facilitator Program, please view the HCF website.
- Is the Leave of Absence form still required for employees requesting a leave?
A. Yes, the Leave of Absence form is still required for all leaves. This form is generally completed by the supervisor or HR representative in coordination with the employee requesting the leave. Once completed, it should be sent to the BRC/Payroll (exception: employees of OGC, EAP, and UC Press send these forms to their own HR dept.)
- How do I request a Liberty Mutual Disability Claim Packet?
A. Contact the main benefits office at 415-476-1400 or email benefits@ucsf.edu, though soon you will be able to submit your claim online.
- Who sends me my COBRA packet if I separate?
A. CONEXIS our new COBRA administrator will send you the packet once they receive record you have separated from the University.
